Adding a Fallen Firefighter’s Name to the List of the Fallen
The list maintained by Canadian Fallen Firefighters Foundation is for those firefighters who have died in the line of duty. There are two pages on this web site that will give you the information you need to have the name of a fallen firefighter added to the list.
1/ Read the Line of Duty Death Definition. This explains the reasons for which a firefighter will qualify to be added to the memorial list.
2/ Download and complete the current version of the Canadian Firefighters Memorial Ceremony & Recognition application. Be sure to follow the instructions below for completion and submission of the application.
Instructions for Submission of the Canadian Firefighters Memorial Ceremony and Recognition Application
IMPORTANT: Applications must be completed in full using the current version of the form ONLY. Incomplete applications or applications submitted using an outdated form will not be processed.
The current version of the application form is Version 3-0114 (3 pages). The version number of the form appears in the lower right hand corner of the last page. If you are unsure if you have the correct version, you may download the current version by following the highlighted download links below.
All applications must include copies of sufficient documentation to validate the application. This may include copies of newspaper articles, letters from the department/association, etc. Circumstances surrounding the death must be clearly indicated on the application. In the case of a death as a result of a workplace illness, a copy of the WCB/WSIB/CSST/VAC claim acceptance letter MUST be included with the application.
For your convenience, we have made the current application form available to you in 2 formats. Both are PDF versions that require the free Adobe Acrobat Reader to view. You may use either the Electronic Version (preferred method) that will allow you to download the form and complete it on your computer before printing it out, or the Manual Version in which you can download, print out and complete by hand. If using the Manual Version, please ensure the form is completed in a legible manner.

Completed applications including all supporting documentation must be submitted by mail to the office of the Canadian Fallen Firefighters Foundation:
